ETA is seeking to extend the following forms without changes: Form ETA-9141, Application for Prevailing Wage Determination ; Form ETA-9141, General Instructions ; Form ETA-9141, Appendix A, Request for Additional Worksite(s) ; Form ETA-9165, Employer-Provided Survey Attestations to Accompany H-2B Prevailing Wage Determination Request Based on a Non-OEWS Survey ; and Form ETA-9165, General Instructions .
The FRN informs the public of the request for three-year approval of the information collection under OMB Control Number 1205-0508 and invites comments from the public for 60 days. Written comments must be submitted in accordance with the Federal Register notice instructions. The deadline to submit comments is Tuesday, May 26, 2026.
