Receipts, approvals and denials are different processing events and are not used here to calculate individual approval rates. Pending and awaiting-visa inventory are stocks, not quarterly flows. Missing or suppressed values are not treated as zero. Cumulative data and quarterly flows are kept separate; O-class totals are not O-1-only data.
